Instructions for filling in EASA Form 19


EASA Form 19 should be completed as follows. Please fill in your personal details at the
top of the form. Then choose the right alternative by ticking the relevant box: do you
apply for an initial aircraft maintenance licence, for amendment or renewal of your
Part 66 licence, or for conversion of your national maintenance mechanic licence into
a Part 66 licence.

Block 1

Enter here the details of your JAR-66 licence (if applicable). Please complete this block also when applying for amendment or renewal of your JAR-66 or Part 66 licence.

Note: If you apply for amendment or renewal of your JAR-66 licence, the licence will automatically be converted into a Part 66 licence. A national licence can be converted by filling in the appropriate boxes on this form.

Block 2

Enter here the details of your Finnish national maintenance mechanic licence to be converted, even if the licence is no longer valid.

Block 3

Enter here the details of your employer, if the employer applies for the licence on your behalf and the licence should be sent to the employer’s address. If you apply for the licence yourself, leave this block empty.

Block 4

Tick those boxes which correspond to the licence categories or subcategories applied for. Category B1 covers category A, and thus category A need not be ticked when applying for category B1.

Block 5 (2 alternatives)

Enter here the type endorsements applied for. Mark the powerplant as in your type training certificate. In case the powerplant is not indicated on your certificate, please attach the training programme or another document showing the type of powerplant included in that type rating course.

Note: If you note some differences when entering the type endorsements from your JAR-66 licence, e.g. in the markings used on the certificate, please write the different marking in brackets after the type endorsement copied from the licence and attach the training certificate to your application.

Block 5 alternative 1: When applying for conversion of national licence

When converting your national maintenance mechanic licence into a Part 66 licence, in case you also hold a JAR-66 licence (which is also converted with the national licence), enter here the type ratings and powerplant endorsements contained in your JAR-66 licence. Write the type ratings contained in your national licence to be converted in block 6.

Block 5 alternative 2: When applying for amendment of a valid licence

This block 5 is used when applying for amendment of a valid JAR-66 or Part 66 licence. Do not fill in this block when applying for licence renewal, unless you also apply for an amendment to your licence, e.g. a new type rating.

Note: The licence category for which the training was given must also appear from the training certificate, or the issue must be attested by some other official certificate received from the training organisation. You may also enter the licence category in this block in addition to the above mentioned training information. However, this is not sufficient alone, but a certificate from the training organisation is still required.

Block 6

Use this block only when converting a national maintenance mechanic licence into a Part 66 licence. Copy here the type endorsements contained in your national licence.

Since the Part 66 licence contains a powerplant endorsement in addition to the type endorsement, the powerplant endorsement should be checked from the original training material, training certificate or attested with a separate certificate issued by the training organisation or employer. If the powerplant in question and its exact marking do not appear from the application, the type endorsement cannot be entered in the Part 66 licence.

Note: If you note some differences when entering the type endorsements from your national licence, e.g. in the markings used on the certificate, please write the different marking in brackets after the type endorsement copied from the licence and attach the training certificate to your application.

Block 7

Specify here the attachments sent with the application by ticking the relevant box or by writing the number of attachments in the empty space (e.g. ”attachments: 8”). The attachments must be original documents or certified copies.

Block 8

The title of this block contains a text with alternative words to complete the sentence. Please strike out those words which are not applicable. Before signing the form, make sure that you have filled in all necessary boxes on the form and that you understand the meaning of the written assurance given in block 8.

Bottom of page 2

There are check boxes with choices at the bottom of the page under block 8. Please tick the appropriate box(es). An English version of the licence can be requested by ticking the relevant box; if you leave this box empty, you will get a licence with Finnish and Swedish text. Choose also how you want your licence to be delivered.

Block 9

Maintenance experience can be recorded here, if you have not received a separate certificate from your employer. However, all entries must be signed by the employer’s representative.

If you do not fill in block 9, please remove the last page (3) of the form.

Block 10

This block should be left empty.

Note: This block is contained in the original version of EASA Form 19, and it is used in those countries where maintenance organisations are allowed to write licences. In this case the organisation’s representative signs the recommendation, but he/she still has to send the application form to the authority for final approval. In Finland, this block is not in use for the time being.
